Frenulotomy

Frenulotomy, also known as a frenectomy, is a simple surgical procedure performed to correct a congenital condition in which the lingual frenulum (the small fold of tissue under the tongue) or the labial frenulum (the fold of tissue that attaches the upper or lower lip to the gums) is too tight or short, causing restrictions in movement. This condition is often referred to as being “tongue-tied” or “lip-tied”.

Indications for Frenulotomy
Lingual Frenulum: When the lingual frenulum restricts tongue movement, potentially affecting speech, eating, and, in infants, breastfeeding.
Labial Frenulum: A tight labial frenulum can cause a gap between the front teeth, receding gums, and issues with oral hygiene.


Evaluation: A thorough examination to assess the need for the procedure.
Anesthesia: The procedure is often performed with local anesthesia, especially in older children and adults. In infants, it may be done without anesthesia or with a topical numbing cream.
Cutting the Frenulum: The frenulum is snipped with sterile scissors or a laser, which frees the tongue or lip from its restricted position. The process is usually quick, with minimal bleeding.

 In some cases, a laser may be used instead of scissors, which can reduce bleeding and discomfort during and after the procedure.
Recovery and Postoperative Care
Minimal Discomfort: The procedure typically results in minimal discomfort, and pain relief is rarely needed.
Feeding: Infants can breastfeed immediately after the procedure, which can help soothe them and aid in healing.
Oral Exercises: Stretching exercises may be recommended to prevent the frenulum from reattaching.
Healing: Complete healing usually occurs within a few weeks.
Risks and Considerations
Bleeding and Infection: Minimal risk of bleeding and infection.
Reattachment: There’s a possibility of the frenulum reattaching, requiring repeat procedures.
Effectiveness: While frenulotomy can be very effective, especially in improving breastfeeding in infants, its impact on speech development is less clear and may require additional speech therapy.
Frenulotomy is generally a safe and straightforward procedure with a quick recovery time. It can significantly improve the quality of life, especially for breastfeeding infants and individuals with speech difficulties. However, it’s essential to have a proper evaluation by a qualified healthcare provider to determine if the procedure is necessary and appropriate.
